Adult Mental Health

Independent Management Services offers two key programs to support adults living with serious mental health conditions: Adult Rehabilitative Mental Health Services (ARMHS) and Adult Mental Health Targeted Case Management (AMH-TCM). ARMHS focuses on helping individuals build and strengthen skills to manage symptoms, improve daily functioning, and increase independence. AMH-TCM connects clients with essential resources, coordinates care, and provides ongoing support to promote stability and help individuals achieve their personal goals. Both services are designed to empower adults to lead healthier, more self-sufficient lives.

Adult Rehabilitative Mental Health Services (ARMHS) are rehabilitative services designed to help individuals develop and strengthen skills necessary for psychiatric stability, social functioning, emotional adjustment, and independent living. These services are intended for individuals whose abilities have been impaired by symptoms of mental illness. ARMHS also supports individuals at risk of losing significant functioning or being placed in more restrictive settings, helping them maintain stability and independence.

Services may include instruction, assistance, and support in areas such as:

  • Medication education and monitoring
  • Symptom management
  • Basic social and living skills
  • Household management
  • Employment-related and community living transitions

ARMHS practitioners are committed to a collaborative, person-centered approach. They work closely with each client to develop an Individual Treatment Plan (ITP) tailored to the client’s unique goals, strengths, and needs. These plans are created in partnership with the client to ensure relevance and foster ownership of the recovery journey.

Services are delivered in home and community-based settings, enhancing accessibility and allowing clients to apply skills in real-life environments. By meeting clients where they are, ARMHS practitioners support meaningful progress toward mental health stability and independent living.

Adult Mental Health Targeted Case Management (AMH-TCM) is a service designed to support adults diagnosed with a Serious and Persistent Mental Illness (SPMI) by helping them access and coordinate essential services that promote stability and recovery. AMH-TCM consists of four core components: Assessment
  • A comprehensive evaluation is completed to assess the individual’s current level of functioning and how it is impacted by their mental illness.
  • The assessment identifies strengths, needs, preferences, and goals and serves as a foundation for the development of the Individual Community Support Plan (ICSP).
Planning
  • Based on the assessment, a person-centered, ICSP is developed in collaboration with the individual outlining the person’s mental health and life needs – such as housing, employment, education, physical health, etc.
  • The plan is reviewed regularly to reflect changes in the individual’s circumstances and/or goals.
Referral and Linkage
  • The case manager connects the individual with appropriate formal services (e.g., mental health therapy, psychiatric services, substance use treatment) and informal/natural supports (e.g., family, peer supports, faith communities).
  • Linkages are made to community resources such as housing programs, employment services, and benefit assistance.
  • The goal is to reduce barriers to care and ensure the individuals receive the support necessary to meet their goals.
Monitoring and Coordination
  • The case manager maintains regular contact with the individual to monitor progress toward their goals and evaluate the effectiveness of services.
  • Services and needs are assessed and reassessed regularly to ensure they remain relevant and adequate.
  • The case manager helps identify emerging needs, address challenges, and modify the ICSP as necessary.
  • Coordination among service providers is facilitated to ensure continuity of care and avoid duplication of services.
Who is eligible to receive ARMHS?

To be eligible for ARMHS, MHCP members must meet all of the following criteria:

  • Be 18 years of age or older.
  • Have a primary diagnosis of a mental illness as determined by a diagnostic assessment.
  • Have a completed level of care assessment.
  • Have significant impairment in functioning in three or more areas of the functional assessment domains as specified in statute.

To be eligible for Adult Mental Health Targeted Case Management (AMH-TCM) in Minnesota, an individual must meet the following criteria

  • Be 18 years of age or older.
  • Be enrolled in Medical Assistance (MA).
  • Have a diagnosis of a Serious and Persistent Mental Illness (SPMI), as defined by Minnesota Statutes.
  • Demonstrate a need for service coordination to maintain or improve functioning in the community.
  • Initial eligibility is based on a diagnostic assessment (DA). A DA is completed annually to determine member’s continued eligibility for case management services.

Medical Assistance (Medicaid) or MinnesotaCare coverage is required to access ARMHS and AMH-TCM. If you’re unsure whether you qualify, please contact our intake coordinator at 507-437-6389 for assistance.

ARMHS (Adult Rehabilitative Mental Health Services) may be needed if the individual:
  • Has a mental health diagnosis and is experiencing functional impairments in daily life (e.g., managing a household, social skills, employment readiness).
  • Needs skill-building support to manage symptoms, live more independently, and improve daily functioning.
  • Is motivated to work on personal goals like symptom management, housing stability, or employment.
  • Would benefit from in-home or community-based support focused on practical, real-world skills.
Think of ARMHS as hands-on, skill-building support.

AMH-TCM (Adult Mental Health Targeted Case Management) may be needed if the individual:
  • Has a Serious and Persistent Mental Illness (SPMI).
  • Needs help navigating complex systems (e.g., housing, healthcare, benefits, legal services).
  • Requires ongoing coordination of services to stay stable in the community.
  • May be at risk of hospitalization or homelessness without coordinated support.
  • Needs a case manager to advocate and connect them with resources.
Think of AMH-TCM as a guide and coordinator for long-term stability.

Some individuals may benefit from both services:
  • For example, someone with SPMI who needs both case management and skill-building support may qualify for both AMH-TCM and ARMHS.
