Adult Mental Health
Adult Mental Health
Independent Management Services offers two key programs to support adults living with serious mental health conditions: Adult Rehabilitative Mental Health Services (ARMHS) and Adult Mental Health Targeted Case Management (AMH-TCM). ARMHS focuses on helping individuals build and strengthen skills to manage symptoms, improve daily functioning, and increase independence. AMH-TCM connects clients with essential resources, coordinates care, and provides ongoing support to promote stability and help individuals achieve their personal goals. Both services are designed to empower adults to lead healthier, more self-sufficient lives.
Adult Rehabilitative Mental Health Services (ARMHS)
Adult Rehabilitative Mental Health Services (ARMHS) are rehabilitative services designed to help individuals develop and strengthen skills necessary for psychiatric stability, social functioning, emotional adjustment, and independent living. These services are intended for individuals whose abilities have been impaired by symptoms of mental illness. ARMHS also supports individuals at risk of losing significant functioning or being placed in more restrictive settings, helping them maintain stability and independence.
Services may include instruction, assistance, and support in areas such as:
- Medication education and monitoring
- Symptom management
- Basic social and living skills
- Household management
- Employment-related and community living transitions
ARMHS professionals are committed to a collaborative, person-centered approach. They work closely with each client to develop an Individual Treatment Plan (ITP) tailored to the client’s unique goals, strengths, and needs. These plans are created in partnership with the client to ensure relevance and foster ownership of the recovery journey.
Services are delivered in home and community-based settings, enhancing accessibility and allowing clients to apply skills in real-life environments. By meeting clients where they are, ARMHS professionals support meaningful progress toward mental health stability and independent living.
Adult Mental Health Targeted Case Management (AMH-TCM)
Adult Mental Health Targeted Case Management (AMH-TCM) is a service designed to support adults diagnosed with a Serious and Persistent Mental Illness (SPMI) by helping them access and coordinate essential services that promote stability and recovery.
Here’s a summary of what AMH-TCM does for individuals with SPMI:
- Assessment: Case managers evaluate the individual’s needs, strengths, and challenges to develop a comprehensive understanding of their situation.
- Planning: They create an Individual Community Support Plan (ICSP) that outlines goals and the services needed to achieve them.
- Coordination: Case managers help individuals connect with and navigate a wide range of services, including:
- Mental health treatment
- Medical care
- Housing support
- Employment and vocational services
- Educational resources
- Social and recreational opportunities
- Legal and advocacy services
- Monitoring: They regularly check in with clients to ensure services are effective and adjust plans as needed.
- Support: Services are person-centered and often delivered in the home or community, making them more accessible and relevant to daily life.
The goal of AMH-TCM is to help individuals with SPMI achieve greater stability, independence, and quality of life by ensuring they have consistent access to the resources and support they need.
